This is for the established patient that is not part of the Monthly membership. During this follow up appointment, your progress will be discussed and any labs will be reviewed along with reviewing your medications, supplements, nutrition, and lifestyle.

Cancellation Policy

To cancel or reschedule, please contact at least 8 hours in advance to avoid being charged for missed appointment time. If there is a specific time you are looking for that is not on the calendar, please reach out to me.
